1. Start by cutting out the table/chart around the perimeter.

2. Fold the left side inward so that the edge meets up with the line between the first and second columns, as shown in the first photo below.

3. Continue/trace the horizontal lines to draw the table on to the back side of the chart that you folded over and label the new column “Visual” as shown in the second photo.

4. Complete the information in the table, including both inside columns and the one on the outside. Use the information from the picture labeled 'Notes for the chart.

5. Glue the back of the first two columns only and place it into the notebook. The third column/backside should swing open freely.
